CSAT QR CODE SURVEY

Collect CSAT feedback with a QR code

Ask customers to rate a specific interaction from 1 to 5 while it is still fresh. One CSAT campaign can power separate QR placements on receipts, tables, counters and packaging, giving the team a clear score, the reason behind it and a workflow for low responses.

QUICK OVERVIEW

What this changes in practice

01

Use CSAT for one identifiable interaction

Customer Satisfaction Score is a direct measure of how satisfied someone was with a specific experience. Ask after the visit, collection, delivery, purchase or support case — close enough that the customer knows exactly what they are rating.

Phrase the question around that moment: “How satisfied were you with today’s visit?” is more actionable than “How satisfied are you with our company?” The broader relationship question belongs to NPS.

02

Build the campaign once, then split placements

Create the CSAT campaign first. It defines the 1–5 question, optional follow-up, language, branding, low-score rule and notification workflow.

Add a separate QR placement for every surface you may want to compare. A receipt and table card can open the same survey while preserving their own scans, views, starts and submissions.

  • One shared question for consistent measurement
  • A unique QR code for each physical touchpoint
  • Placement-level conversion before score comparison

03

Keep the survey short enough for the moment

Lead with the 1–5 satisfaction score, then ask one reason as a choice or short text. Add optional contact details only when someone can follow up. A concise survey is easier to complete beside a till, at a table or during unboxing.

Mosaqo supports up to ten questions, including choices, text, smileys, thumbs and scales. Treat that as flexibility for the campaign, not a target for the customer.

04

See whether the prompt works before judging the score

The dashboard shows the average CSAT score and response distribution alongside the feedback funnel. Scans show visibility, starts show intent and submissions show completion. Read these together before deciding that one location performs better than another.

A low response count can point to weak placement or unclear copy. A repeated low score with a recurring reason points to the experience itself. Separate QR placements make those two problems easier to distinguish.

05

Route low CSAT responses to an owner

Choose a low-score threshold and alert recipient before launch. Every response arrives in the shared inbox with the campaign, placement, answers and form version, ready to assign, resolve or close.

Customers can respond anonymously. When they voluntarily add contact details and follow-up consent, the team can contact them directly without making identification a condition of giving feedback.

06

Test the question at the physical touchpoint

Place the code after the experience being measured and name that experience beside it. “Rate your pickup today” creates a clearer expectation than “Scan for survey”.

Scan the final printed proof on a real phone, complete a test response and verify the low-score workflow before the campaign goes live.

FAQ

Frequently asked questions

What does a CSAT QR code measure?

It opens a short satisfaction survey for a specific interaction. In Mosaqo, the dedicated CSAT question uses a 1–5 scale and can be followed by a reason or optional contact field.

Where should I place a CSAT QR code?

Place it immediately after the experience: on a receipt, bill folder, pickup counter, delivery insert, support follow-up or event exit. The prompt should name what the customer is rating.

Can one CSAT campaign compare several locations?

Yes. Use one campaign with a separate placement for every location or surface. They share the same question while retaining their own QR codes, funnels and response breakdowns.

Can I get an alert after a low CSAT score?

Yes. Configure a low-score threshold and notification rules for the campaign, then manage the response in the shared inbox.
